Release v0.5.4 — GH #209 audit fixes (correctness, panics, perf)

Whole-codebase 2x2 audit follow-up to the GH #209 fix. 13 findings
fixed, 1 (bounded raw-HTML tag scan) deferred to BACKLOG.

Correctness / panics:
- Split-title displayed-math chains no longer O(n^2) or stack-overflow
  (SIGABRT that Tine's per-page catch_unwind can't catch); unclosed-math
  tails after a chain are now owned instead of panicking (#209 class).
- latex-env same-line tails compose as blocks (were demoted to inline).
- org whitespace-only headline in a callout body coalesces to one plain
  (was a fuzz-floor breach).
- refs: entity file-label no longer indexes a phantom page; org inline
  footnote definitions contribute their block refs.
- cookies/timestamps reject >32-bit ints (mldoc jsoo %d parity).

Perf (constant-factor, per-block hot path): hiccup closer index without
the O(H log H) sort; shared quote-body scan; hoisted/lazy allocations.

Harness hardened to fail closed on oracle errors and isolate vdiff_iso.

Gates: run.mjs 1402/1402 + audit 7533/0; lib/render/complexity/perf/
census; fuzz floor 0 (md+org).
