Add automatic session refresh and per-account cookie storage

Sessions now auto-refresh when expired by silently loading claude.ai
in a hidden window. Added "Refresh Session" tray menu option that
falls back to opening a login window (updating the existing account
in place rather than creating duplicates). API calls now send all
cookies per-account instead of just sessionKey, fixing auth issues
with Claude's updated requirements.
