Entity list added to diagnostics report

The diagnostics download now includes a list of all entities registered to the
integration, each with its entity_id, domain, current state, and whether it is
disabled. This makes it easier to diagnose availability issues from a bug report
without needing to ask for a manual description of the user's setup.
