Added proportional Map Note markers using their original Foundry icons.
Added the Show Map Notes client setting, disabled by default.
Added Map Note hover text showing the custom Note text or linked Journal title.
Added Ctrl+S support for opening linked Journal Entries and Journal Entry Pages.
Added Map Note visibility handling for permissions and Fog of War.
Added automatic redraws when Map Notes are created, updated, refreshed, or deleted.

Documentation and Testing

Reworked the README with clearer installation, configuration, usage, and feature documentation.
Added Map Note unit tests.
Added the Show Map Notes localization key to all 37 language files.
Maintains compatibility with Foundry VTT 14 and is verified with Foundry VTT 14.367.
