Add Windows executable and macOS distributions for v1.0.4

• Windows: AI2SQL-Generator-Portable-1.0.4.exe
• macOS:
  • AI2SQL-Generator-1.0.4.dmg (Disk Image)
  • AI2SQL-Generator-1.0.4-mac.zip (ZIP archive)

These are the distributable files for the latest version of AI2SQL Generator.
Users can now download and run the application directly without needing to build from source.

Windows users:

• Download and run the .exe file.

macOS users:

• Option 1 (DMG): Download the .dmg, open it, and drag the application to your Applications folder.
• Option 2 (ZIP): Download the .zip file, extract it, and move the application to your Applications folder.

The ZIP file is provided as an alternative distribution method for macOS users who prefer it or have issues with the DMG format.
