问题修复

• 修复工具失败信息重复显示、已恢复的 OpenCode 响应仍被判定失败，并仅为最近一次失败保留重新编辑入口。
• 修复共享开关在部分设置布局中尺寸被文本输入样式覆盖的问题。

• Fixed shared switches inheriting text-input dimensions in some settings layouts.
