- Updated to duroxide 0.1.1 API
- fetch_orchestration_item/fetch_work_item: add poll_timeout parameter
- RuntimeOptions: dispatcher_idle_sleep -> dispatcher_min_poll_interval
- Updated continue_as_new() calls to new awaitable API

Note: test_worker_lock_renewal_extends_timeout may fail with high-latency DB connections
