Renaming Copilot Chat Sessions Doesn't Stick

Type: Bug

1. When attempting to rename an existing Copilot Chat Session, upon submitting the new name, its entry in the Sessions list doesn't automatically update, but rather requires a manual refresh for the new name to appear.

2. Entering into the renamed chat session, the old name still appears at the top beside the back arrow.

3. Upon backing out of the chat, it again appears under the old name in the Sessions list. The same occurs if actively in the renamed chat, exiting VS Code and relaunching -- it's back to the old name in the list.

2. Click Rename
3. Give it a new name
4. Press Enter
5. Rename does not apply
6. Click Refresh Sessions
7. Rename applied
8. Open renamed session
9. Session name reverts to original
10. Exit session
11. Session name reverted in list
