MCP app flow for github pr is not polished

Issues

• I told the agent to create a PR, and I'm in "bypass approvals" mode, so I don't want to see a PR form at all- it's like a tool confirmation, which I disabled. I want it to work autonomously
• The MCP app seems to not show up when it was creaed with the session in the background, so I have to ask teh agent to try again using the gh cli
• the form has an unclickable link

• I have strange attachments after submitting the form

• Tool call is rendered as "failed" when the form is shown

• Showing me this just seems unnecessary

The rest are GH MCP things. I made a PR for the MCP server so we can turn of deferral forms: github/github-mcp-server#2921
