test: commenting for exit plan mode

This iteration, we switche the plan widget commenting feature back to the editor. In Agents window, there is simply way more real estate for this to happen. In the editor window, opening it up to the side works as well.

Additionally, with the new Markdown extension rendering, it looks visually much better and easier to read.

Testing:

1. In Agent Host, create a plan in Plan mode.
2. Once the plan is finished it will prompt you approve that plan somehow.
3. Click on Open full plan and that will allow you to add some comments.
4. Select text to comment.
5. Submit in the bottom right to approve those comments and continue session.

The location of Plan mode changed (a while ago), but I hadn't used it. Click "Interactive" and then "Plan" mode should be under there:

Cool feature, I like being able to modify the plan!
