Fixes

Fixed an issue where reasoning output from DeepSeek models run through the Codex runtime could fail to pass back correctly, resulting in incomplete or missing reasoning traces during chat. This release also includes internal improvements to failure diagnostics to help us catch and resolve chat issues faster going forward.
