Important account-isolation fix

• Give each device-login credential source its own stable login lineage instead of deriving identity from the shared Kiro profile ARN. Fixes #86 via #88. Thanks @st333in for the report and patch.
• Automatically rebind legacy profile-derived identities on load. The review follow-up ensures migrated internal credentials remain usable and that replaced credentials are still rejected.
• Group banned dashboard accounts below paused accounts (#83).

Upgrading

Upgrade from v0.2.2/v0.2.3 to prevent shared login identities. Existing legacy identity bindings are replaced automatically; no manual database edits are required. Credentials already overwritten before upgrading cannot be reconstructed by this migration. Re-add affected accounts if their original credentials were lost.

Validation

The corrected PR passed the full Rust test suite, formatting, Clippy, frontend validation, and Linux/Windows x64/ARM64 builds. Release assets are published only after the tagged source passes the release workflow and all four downloaded artifacts pass SHA256 verification.
