Setup Command

• npx claude-cortex setup — Injects proactive memory instructions into ~/.claude/CLAUDE.md so Claude uses memory tools consistently across all projects. Idempotent (safe to run multiple times).

Hook Subcommand (fixes #1)

• npx claude-cortex hook pre-compact — Run pre-compact hook
• npx claude-cortex hook session-start — Run session-start hook
• Fixed documentation that referenced non-existent claude-cortex-hook npm package
