v1.0.10: security and robustness fixes

- Remove hard-coded GitHub token; read AHP_GITHUB_TOKEN from wp-config.php
- OTP rate-limit window uses the site clock instead of MySQL NOW()
- Invoice sequence number is now atomic (LAST_INSERT_ID)
- Issuing an invoice claims the cart atomically and rolls back a partial
  invoice (restoring the cart) if a line item fails to save
- wp_unslash cart and login input (no more backslashes in descriptions)
- Per-IP limit on OTP requests (filter ahp_otp_ip_limit)
- Daily cleanup of old OTP rows and empty guest carts
