Release 1.6.0

Commit: Streamline macOS deployment: fix config file resolution and installer

Rework how refinify.lua locates its secrets/prompt files (now
~/.config/refinify/, resolved via $HOME instead of the symlinked script's own
directory) so the system prompt and API key actually load instead of
silently falling back to defaults. The installer now seeds/migrates that
config directory automatically, waits for Hammerspoon to actually finish
installing and launching, walks the user through granting Accessibility
permission, and reloads Hammerspoon so hotkeys work immediately - closing
the gaps that left a colleague's install broken with no manual recovery
path. Windows install is untouched.
