Changes

• Keep successfully saved attachments in the draft when other files fail to save, and report which files failed.
• Preserve streamed responses and pending guidance when loading conversation history or updating automatic approval reviews.
• Keep messages in conversation order when saving an archived thread as a Markdown note.
• Prevent archiving a thread that becomes active while its Markdown export is being saved.
• Reload visible settings resources after changing the Codex executable.
