Mirror Model Subpackages In REST Services

• Added the opt-in Maven parameter useModelSubPackage for queryable:source (V3) and queryable:sourcev4 (V4), defaulting to false.
• When enabled, REST packages and source paths mirror the full relative model subdirectory, including arbitrary names, multiple levels and custom model/REST roots. Root entities and service class names are unchanged.
• Automatically relocate existing services when switching between flat and mirrored packages, preserving manual methods, exclusions and V3 preserved-filter hooks. Conflicting copies fail explicitly without being overwritten.
• Keep application constant imports and HTTP resource paths unchanged; V4 services now import the entity's declared package. OpenAPI source lookup uses the same service-location rules.
• V4 generation failures now propagate to the Maven goal rather than reporting success after logging an exception.
• Added regression tests for both versions, default behavior, relocation in both directions, repeat generation, conflicts, custom roots and generated Maven parameter descriptors.
• Added real Quarkus/H2 HTTP and CDI checks for relocated V3 services (with and without OIDC) and V4 services with custom roots and nested entity packages.
• Updated README, agent guide and GitHub Pages to 3.0.12, including configuration and migration notes.
