Credentials form does not appear in version > 1.111.1

Bug Description

Credentials form does not appear in version > 1.111.1

I update my docker stack to 1.112.6 and 1.113.3, but credentials form does not appear.
in new and update credentials.

To Reproduce

1. clic in credentials
2. select app or service ex. OpenIA
3. form inputs not appear.

Expected behavior

aways appear form to input API Key, Organization ID (optional), Base URL, Allowed HTTP Request Domains etc.

Debug Info

i test in all browsers. clean cookies and cache.

1.112.6 or 1.113.3

22.16.0

in 1.114.0 credentials form it's ok.
