Fixes found while auditing v0.1.0 before release.

--json was dropping most of the answer

silence, map and changes answer mostly in prose, not rows. The JSON
envelope only carried rows, so a consumer got an exit code with nothing in it:

Worse, a failed diff returned exit 64 with an empty envelope and no reason.
QueryResult now carries text — the answer as rendered for a terminal:

Anything branching on rows keeps working. Anything that needs the reasoning no
longer has to run the command a second time without --json.

Smaller

• void return types were recorded as an external dependency and turned up in
  unresolved as Void.
• impl printed the file path twice when a type is registered in the file that
  declares it. Now @ line 14.
