Fixes the prep-to-health transition so primer and primary warmup operations receive the handoff cancellation token. Foreground health lanes now start immediately after the bounded grace period even when a provider operation is slow to cancel, instead of waiting for the primer timeout.

Late speculative cleanup remains constrained by the shared connection budget.
