Blockchain Scalability Book 1.1.1

This patch fixes the PDF's front matter and navigation.

  • Adds a cover, title page, and copyright/version page in standard order.
  • Adds a six-page printed table of contents for chapters and main sections, with verified page numbers and clickable entries.
  • Uses lowercase Roman numbering for front matter and restarts Arabic numbering at Chapter 1.
  • Adds running headers and page footers, with no page number on the cover.
  • Adds a fully nested 880-entry PDF bookmark outline covering headings through level 3.

The attached PDF and HTML archive were built from source commit 1c0e3c9564af570ea9b8aba8923ebe36d0033ed5. Automated checks confirmed 390 printed TOC destinations, all 880 bookmark destinations and hierarchy, 0 replacement characters, and 0 blank pages. Representative front-matter, TOC, chapter-opening, and mid-book pages were visually inspected.
