Client Feedback Implemented (Ali SDS)

  • Removed: NDIS Number field (Step 1)
  • Added: Gender / Sex radio field (Male, Female, Other, Prefer not to say)
  • Updated: Date of Birth is now typeable (DD/MM/YYYY format) with calendar picker button
  • Removed: Plan Start Date and Plan End Date (Step 4)
  • Removed: Medical & Support Information step entirely (Primary Disability, Mobility, Communication, Behavioural Needs, Allergies)
  • Form reduced: 8 steps → 7 steps
  • Confirmed: Mobile number is mandatory
