DNS target filtering and configuration management update.

- Add dns.blocked_hosts with exact and wildcard domain matching before routing, DNS, and outbound connection setup.
- Return HTTP 403 and SOCKS5 REP 0x02 for blocked targets without issuing DNS queries.
- Display blocked host counts and patterns alongside loaded route rules.
- Include not-setup in the generated example configuration.
- Rename --open-config to --reveal-config and reveal the selected file in Finder with open -R.
- Add migration errors, tests, and documentation for the new configuration and CLI behavior.
