Hardening patch: malformed embedded source-hygiene policy now degrades safely instead of panicking; unreachable helpers and current Go 1.26 staticcheck findings were cleared without changing retrieval scoring or CLI behavior. Build, vet, normal/race tests, current staticcheck, zero-critical/high Go footgun audit, and VS Code extension compile passed.
