BREAKING CHANGES

• NbPagination size="sm" is 32px tall (was 40) and
  size="lg" is 64px (was 56), matching NbDataTable. size="md" is unchanged,
  so only applications passing sm or lg are affected.
• NbShell moves the notification, menubar and topbar regions
  out of .nb-shell__body and above .nb-shell__content-row. Applications
  that style or query those regions by their position in the shell's DOM need
  updating; the class names, slots and public API are unchanged.

• the default field fill has changed. Enabled fields were
  #cacbcc in light, a mid grey that sat closer to the disabled fill than to
  the page and made "you can type here" hard to read; they are now #f1f2f3,
  with the old value kept as the disabled fill. Dark follows the same shape.
  Applications that want the previous treatment can import the
  styles/presets/classic-fields opt-out, which restores it exactly.
