Major release with a new .ccv container, rewritten library API, and a new cross-platform CLI tool.

Breaking changes

• Project renamed from CCVC to CVC.
• The old ConsolePlayer, Converter, and Installer projects were replaced by the new cvcutil CLI.
• .ccv format was updated to CVC2 with separate container and codec versions.
