Click count refactor, dashboard top domains, and icon fixes
Click counts from aggregate queries: click_count on slugs is now computed via a subquery against the clicks table rather than maintained as separate link_click_count and qr_click_count counter columns. This keeps counts consistent with the analytics data and removes the need to update counter columns on every click.
Dashboard top domains: The "Top Sources" panel is renamed "Top Domains" and now groups by referrer_host instead of the full referrer URL, giving cleaner domain-level attribution.
iOS and macOS icons: The OS breakdown icons for iOS and macOS now use valid Material Symbols glyphs (phone_iphone and laptop_mac) instead of the broken apple ligature.
