ui(v2.3.0 polish): unify focus-ring contract + 44px touch-target token

- Single :focus-visible contract: outline 2px solid primary, offset 2px,
  no box-shadow. Drops the double-shadow ring on icon-btn / cat-tab /
  format-option / pill-option / btn-primary / btn-secondary / ws-btn /
  close-btn* / pagination-btn. New .btn-tertiary, .floating-card-surface,
  .toolbar-primary selectors join the list. Outline-only is more
  consistent across components and works under forced-colors.
- New --touch-target token (2.75rem). .close-btn-md bumped 2.5rem to
  2.75rem under (any-pointer: coarse) to hit WCAG 2.5.5.
- Resume popup copy simplified: N PDF * M pages, drops the "Undo history
  will reset" and "custom watermark" hints. The summary already conveys
  the load on next render.
- Mobile toolbar primary buttons (Merge / Watermark export / Extract /
  Export PDF) and ws-page-plus inherit the unified focus ring via the new
  .toolbar-primary / .floating-card-surface classes.

No CHANGELOG bump, v2.3.0 entry already covers the user-visible behaviour.
