Fixes and polish

• Recover activity dots that keep running after a task finishes when its Stop hook is missed. The widget checks tracked turns every 15 seconds through the local Codex app-server and clears activity only when the exact turn has a terminal status and a recorded completion time. Added by @ognjeeen in #11.
• Keep activity visible for other running sessions and prevent a late completion response from clearing a newer turn. Unknown results, unavailable requests, and timeouts preserve the current activity state.
• Fix a shutdown race between the activity timer and monitor disposal that could interrupt cleanup of the usage monitor and shared app-server session.
• Add regression coverage for missed hooks, parallel sessions, late responses, completion metadata, timeouts, and shutdown.

Automatic recovery requires Codex CLI support for thread/turns/list and a recorded completion time. A hard termination without recorded completion can still require a later lifecycle event or widget restart.
