Bug Fix

System prompt injection no longer depends on MCP detection timing — The before_agent_start hook was gated on cachedStatus.available, but MCP tools load AFTER this hook fires. The comprehensive SKILL.md routing rules never got injected into the system prompt. Now injects unconditionally when enforceRouting or routingInstructions is enabled.
