Bug Fix

Fix context-mode MCP server working directory — context-mode's start.mjs does process.chdir(__dirname), overriding the project cwd that OMP passes. Added bin/ctx-mode-wrapper.mjs that captures the real project directory as CLAUDE_PROJECT_DIR before delegating to start.mjs. Sandbox commands (ctx_execute, ctx_batch_execute) now run in the correct project directory.
