Codex fails to kill processes, run system stuff, even git commit / lock handling inside repo; generally do high-permission activities on Full Access permissions. Auto-review works with escalation notify clearing most of them.

What steps can reproduce the bug?

What is the expected behavior?

Full access same as prev version w/ 5.4.

The upgrade to codex-cli 0.124.0 just ruined everything... on last versions we had to choose full because default was broken.. and now full is broken!

Yes， ruined my whole day life. I had to use --add-dir to make it work.

Please update to 0.125.0 as it fixes a bug with /permissions to full-access not taking effect in a session.
