Codex IDE Extension: latest chat becomes inaccessible after restarting VS Code

After closing and reopening VS Code, the latest Codex chat/session becomes inaccessible.

The chat appears in the history, but clicking it does nothing or opens an empty/stuck view. Older chats may still work, but the most recent session is often impossible to restore.

Expected behavior:
The latest chat should reopen correctly after restarting VS Code.

Actual behavior:
The latest chat becomes inaccessible after restart, causing loss of recent working context.
