Summary

reject active-only reads when a live rollout path points at an archived session
add a final archived guard after fallback rollout resolution, covering stale SQLite active-index paths
add regression coverage for an archived rollout returned from stale active SQLite metadata

Verification

just fmt
cargo test -p codex-thread-store
bazel test //codex-rs/thread-store:thread-store-unit-tests
