This pull request resolves #2760 by keeping realtime follow-up turns from starting until the previous turn is fully finished, while simplifying the local sequencing state into a single response-control phase.
The bug showed up when a realtime output guardrail tripped during an active turn. The session sent a forced interrupt and then immediately sent a follow-up user input, and the transport turned that into another
response.createbefore the server had finished cancelling the prior turn. That allowed the server to reject the follow-up with an "active response in progress" error.
