Confirmation buttons are missing for write tool prompt

I've been playing a bit with the SDK, I tried to use the example from the quickstart (https://developers.openai.com/apps-sdk/quickstart#introduction), the MCP server works, ChatGPT is aware of the tool, but it seems like I'm having an overlay with no confirmation button to accept the write behavior of the tool.

The button simply doesn't exist, what can I do?

This is across the board for people. Talked about here just so you know it's universal and started yesterday.
