v9.16.1

Backport release for Ulmo, cut from the ulmo/lti_fixes_backport branch. It continues from 9.14.3 and carries the LTI 1.3 and LTI Advantage fixes that landed on master between 9.14.3 and 9.16.1. The branch's changelog sections 9.14.6 through 9.16.1 are published together under this one tag; see CHANGELOG.rst for the per-section detail.

Highlights

• AGS: publish a scoreGiven of 0 to the gradebook, reject a non-positive scoreMaximum at the API and the model, and log why a final grade was not published (upstream #701).
• LTI 1.1: report a module_score of 0 from the LTI 2.0 JSON result service instead of omitting it.
• NRPS: limit and page pagination on /context_membership with RFC 8288 Link headers.
• Deep linking: POST the id_token to the tool's redirect_uri; pass the course context claim and the reusable config's deployment_id; fix target_link_uri handling.
• Reusable configs: resolve the effective LTI version from the external config throughout the XBlock runtime paths, and hide the fields in the Studio editor that have no effect.
• Role mapping: LTI 1.3 launches and NRPS use context role URIs and include forum roles like Community TA and Group Moderator.
• AGS line items and results: create line items from resource_link_id when resource_id is absent, fix results URL generation for an optional user_id, allow a blank score comment.

Note for sites on 9.14.5

Published 9.14.4 and 9.14.5 have different contents from this line. 9.14.5 URL-quoted resource_link_id in the launch data (#607); upstream reverted that (#623) and this release follows the revert. Moving from 9.14.5 to 9.16.1 loses that quoting. Tools that depend on the quoted value will see a change.
