docs: v10.1 retroactive README row + CHANGELOG entry

v10.1 was tagged at ec7aef1 but the README version table and CHANGELOG
were never updated to declare the release. Fill in both retroactively
so the docs match what shipped:

- README.md — new v10.1 row above v10.0 (May 8, 2026), one-line summary
  of the 7 commits' scope.
- CHANGELOG.md — new ### v10.1 section organized into 4 topic groups
  (per-agent permission profiles / project trinity Project memory /
  RAG ingestion manifest + hygiene / migration RAG sync). Major
  section header updated to "April–May 2026" to reflect the span.

Notes Procedure 5-S Task C as shipped in v10.1 with a forward-pointer
to its later revert on v11-dev (F-9 fix-follow), so the v10.1 record
stays accurate.

No code change; validate-pack.py PASS (28 checks).
