CCM should ignore nodes for backends that aren't setup yet

What happened?

The CCM is attempting to add nodes as backends that don't have an IP address set. This can happen when the node isn't initialized yet (cloud taint) or if for some reason the node lost its addresses in a failure scenario.

What you expected to happen?

We should prevent updating the backends to have missing addresses.

The issue seems to be here where the node is added without validating IP address. NodeInternalIp returns a "" when IP is missing.

Also, instead of adding each Backend to specific index, it should be appended, so that we can skip the nodes with no Ip address.
