fix(codex): add reconnect action to account cards

Summary

• show a reconnect action for invalid local Codex OAuth accounts
• reuse the Codex reauthentication route introduced by fix(codex): reconnect expired OAuth accounts from chat #472
• keep API-key, healthy, non-Codex, and non-local accounts on their existing flows
• add focused decision-logic tests and acceptance cases

Verification
