Remove the Discord Application Id setting (#52)

The setting was only for Oronder servers older than the /config endpoint,
which reports the server's Discord app id itself. /config has been in the
server since its first public commit, so no such server exists.

The module now always takes the app id and redirect from /config. If /config
can't be read, it falls back to Oronder's own app, so the default server
still pairs through a transient failure. Worlds that stored a value need
nothing done: Foundry ignores settings that aren't registered.
