v2.22.7

Ships cargo-pmcp 0.28.1 (#413), with the follow-ups found by the 0.28.0 live check against
real AWS and GCP. pmcp moves 2.22.6 -> 2.22.7 with no code change of its own, to mint the tag.
No library crate changes. The full notes are in cargo-pmcp/CHANGELOG.md.

Fixed (cargo-pmcp)

• Lambda zips are deterministic (#405), so an unchanged aws-lambda redeploy no longer updates
  the function.
• aws-lambda destroy cleans up after itself. It removes the server's uploaded
  {server}/bootstrap-*.zip artifacts (#406) and the function's /aws/lambda/<fn> log group
  (#407). If either cleanup fails, it warns and prints the command to run by hand.
• A passing post-deploy check prints a one-line summary (#408), and a skipped check says why.
• A native-path rename regenerates untouched stack.ts and app.ts, and app.ts is now
  recorded in the scaffold record (#409).
• Post-deploy exit codes (#410). An endpoint that answers with a 4xx is a failed check (exit 3
  under --on-test-failure fail). 5xx, connection errors and timeouts are infrastructure errors
  (exit 2). warn still exits 0.
• cargo install cargo-pmcp no longer installs the mock_test_binary test helper (#412).
