Bugfix patch

Good afternoon,

This patch release includes fixes for two bugs:

• Axis.Category now properly stringifies tick values (#927).
• Vertical bar plots (of all varieties) no longer forcibly include 0 in their x domain (#942).

In addition, we have updated to the latest version of d3 (3.4.11) and cleaned up the directory structure.
