v1.3.0 -- Setup state for unconfigured plugins

What's New

• Setup state on missing token -- buttons now show "Setup / Open Settings" when the GitHub token is not configured, not just when the repository is missing. This ensures a clear first-run experience when installing the plugin for the first time.
• Token check in refresh cycle -- if the token is removed mid-session, the button gracefully reverts to the Setup state instead of showing an auth error.

Fixes

• Fixed 5 incorrect accent color descriptions in README (Pull Requests, Language, Size, License, Default Branch now match actual colors)
• Clarified README: unconfigured state triggers for both missing token and missing repository

• Added 8 new test cases covering missing-token scenarios across both actions
• All 351 tests passing
