0.2.41: fresh pacing after Codex resets

After a successful Codex "Use reset", MeterUsage discards the account's old pacing estimate. The first fresh quota reading starts a baseline; later usage growth supplies the burn rate and ETA. Baselines survive relaunch and restart when the quota cycle changes or usage decreases.

Redeemed Codex reset credits are removed from archived quota data immediately, so offline refreshes and relaunches cannot offer the spent credit again.
