Fix direct HCI fallback racing on single-adapter systems

The direct HCI adapter fallback was iterating a hardcoded list of hci0–hci3. On systems with only hci0 (standard HAOS), non-existent adapters silently fell back to hci0, causing multiple concurrent connection attempts to the same radio and triggering BlueZ InProgress errors on every restart.

Now enumerates real adapters from /sys/class/bluetooth at runtime, so only adapters that actually exist are tried.
