fix(server): allow settling threads with unanswered async questions

Unanswered async questions prevented users from settling threads they were done with. Manual settlement now dismisses those questions and settles the idle thread in one operation, without sending an answer or restarting the agent.

Running sessions, native input requests, and approvals still block settlement. Automatic settlement still leaves pending questions alone. The existing resolution events clear the question across web, desktop, and mobile.

Validated with 30 focused settlement and snooze tests, server typechecking, and lint. Tests cover dismissal, repeated settlement, and blocked settlement. No client UI code changed.
