Patch: A generated title reaches the sidebar for any session

A session's row carries its title and its utility token totals, and the server
publishes session.updated whenever that row changes. The event was applied
only to the session on screen, so a generated title — or a utility total — for
any other session, or one still to be opened, never reached the sidebar.

The event's row is now patched into the session list for every session, keeping
the sidebar current; only the header and the token view stay scoped to the
active session.

Other changes

• The guides at /docs/ now say what the code-map, file-map, and context tools
  buy you rather than how they work inside, and the deployment guide no longer
  names internal state paths and endpoints a reader has no use for.
