Minor: Project notes arrives gradually

Project notes is now released a step at a time rather than all at once. On an
install where it has not yet been switched on, the feature is simply absent —
no Notes entry in a session's sidebar, no save-to-notes action on a message,
and the notes pages fall through to the ordinary not-found page as though they
were never there. Nothing else changes, and notes already kept are untouched;
they reappear the moment the feature is switched on.

Whether it is on is decided per install and re-checked while the app runs, so
it can turn on without a restart and the interface follows in place.

Minor: A turn's reasoning reaches the memory it writes

Every completed turn is summarised into a short note that later turns read.
That note is built from the agent's final reply, so the reasoning behind a
decision — why one approach was taken and another set aside — was lost once the
reply was written up. The agent now closes a reply that involved real decisions
with a brief "Decisions & why" section, and the memory writer keeps that
section rather than paraphrasing it away. A turn with nothing to weigh in on
leaves it out, so ordinary replies stay uncluttered.

Other changes

• The Plan and Breakdown agents no longer list project notes among the things
  to read when they start, because notes now reach them through the switch
  above instead of being named unconditionally in their instructions.
