fix: persist MSAL cache so Microsoft auth survives restarts

MSAL's token cache is in-memory only — after restart it's empty and
acquireTokenSilent fails. Fix: serialize the MSAL cache after login and
store it in the session. On refresh, restore the cache before calling
acquireTokenSilent. Also short-circuit auth:refresh when the Minecraft
token is still valid (24h lifetime) to avoid MSAL entirely in the common
case.
